Dear Pediatric Endocrine and Diabetes Specialty Family,
As you may know, The Center for Advanced Pediatrics and Yale Medicine, Department of Pediatrics have been collaborating to provide a seamless transition of care for The Center for Advanced Pediatrics endocrinology and diabetes patients.
Currently, Yale Medicine Pediatric Endocrinology provides coverage in Fairfield County at Norwalk and Trumbull with further expansion of services being planned for 2018 at both Norwalk and Greenwich.
In order to accommodate children with Type I Diabetes, Yale Medicine Pediatric Endocrinology has increased office hours at the Trumbull location where you can schedule an appointment with the following providers:

Jennifer Sherr, MD
Kristin Sikes, APRN
Andrea Urban, APRN
There is also a plan to enhance general endocrinology services in Fairfield County in the near future. However, in the interim, general endocrinology will increase office hours at Trumbull to accommodate those children who may need to be seen sooner. The pediatric endocrinologists who will be increasing her availability to accommodate your child's needs is Dr. Anisha Patel.
To make an appointment, you may contact the Yale Pediatric One-Call Scheduling Department at (203) 785-4081. Until your child is seen by Yale Medicine Pediatric Endocrinology, please call The Center for Advanced Pediatrics for refills of your current medication or any questions you may have. If you are choosing to transition care of your child(ren) to Yale, please call The Center for Advanced Pediatrics at 203-229-2000 extension 2095 to have your records transferred.
